Attention deficit/hyperactivity disorder (AD/HD) and borderline personality disorder (BPD) are both complex and significantly impairing mental illnesses which have a great individual and societal impact. AD/HD presents in childhood with emotional, behavioral, and academic impairments, while BPD presents in adolescence or adulthood with dysregulated emotions, interpersonal dysfunctions, unstable self-image, and often recurrent suicidality or self-injury. Research has suggested a high comorbidity between the disorders, possibly due to diagnostic overlap, or an etiological or interactional connection. The existing base of research on the topic of comorbid AD/HD and BPD was systematically reviewed. Fifty-one articles were found to be directly relevant to the comorbidity, while a host of others shared some close relevance. This review found robust support for diagnoses of BPD in around 4-15% of the AD/HD population, while AD/HD was found to be present in around 30-40% of individuals with BPD. In general, individuals with comorbid AD/HD and BPD also displayed more severe symptoms than individuals with either disorder alone. Etiological and interactional theories suggest a biosocial mechanism where AD/HD predisposes individuals to impaired self-regulation, elevated interpersonal dysfunctions, and trauma. Patterns of these dysfunctions and delayed development of emotion regulation skills may increase BPD presentation and symptoms in adolescent and adult years. This comorbidity should be considered when considering outcomes and in treating individuals with either disorder. Clinicians should consider using specialized diagnostic methods and integrating more targeted treatment options.
