Recent literature suggests that lesbians, gay men and bisexuals (LGB) have significant health disparities as compared to heterosexuals on almost all major health indicators in the United States (U.S.). Although the reasons for this are complex and multifactorial, existing research suggests that fear of real or perceived homophobia by health care providers (HCPs) may be a major contributing factor in both accessing and receiving care. Although there is a small body of literature on the attitudes of nurses, physicians and other HCPs towards LGB patients, there is currently no literature exploring the attitudes of the approximately 189,000 nurse practitioners (NPs) in the U.S. Thus, NP's strengths and challenges in providing care to sexual minorities are currently unknown.

This grounded theory study used semi-structured interviews with LGBT experts (n=3), self-identified LGB persons (n=6) and NPs working in primary/outpatient care in NYC (n=19) to develop a conceptual framework entitled Pathways from passive to active intention to provide LGB inclusive care among NPs: barriers and facilitators. The resulting framework reflects the complexity of the multiple, often overlapping and dynamic influences on attitudes and intention to provide LGB inclusive care and includes five factors: 1) NPs baseline passive (i.e. unconscious) intention to provide care to diverse populations, 2) the influence of facilitators to providing LGB inclusive care, 3) the influence of barriers to providing LGB inclusive care, 4) NPs active (i.e. conscious) intention to provide LGB inclusive care versus provision of usual care, and 5) education and experience needed to provide LGB inclusive care. The goal of LGB inclusive care is the reduction of LGB health disparities.

Results of this study conceptually align with previous research and theoretical models, such as Azjen and Fishbein's Reasoned Action Approach, but add significantly to the literature by exploring the nuances of NPs attitudes and attitude formation, conflicts in personal values versus professional obligations, NPs conceptualization of quality care for LGB patients equating with treating them "the same" as heterosexual patients and the individual, family, community and structural barriers and facilitators to providing LGB inclusive care. Implications for practice, education, policy and further research are numerous.
