A review of literature was performed for comparison of diagnostic accuracy between magnetic resonance imaging (MRI) and musculoskeletal ultrasound (MSUS) of rotator cuff pathology in a middle-aged patient. Searches were conducted in March 2016 through PubMed, Medline, and the Cumulative Index to Nursing and Allied Health Literature (CINAHL). From the selected literature five key articles were chosen for further review. Three articles explored the results of both MRI and MSUS in comparison to surgical findings of rotator cuff injuries. Two articles addressed the use of MSUS in assessment of tissue healing post operatively and inter-rater reliability between sonographers of varying experience levels. All studies support use of MSUS as an accurate mode of diagnosis for rotator cuff pathology, comparable and preferred in most cases to MRI secondary to cost difference between the two imaging methods. Being operator dependent, discrepancy was shown between sonographers in results of a shoulder MSUS exam. Further research is needed to explore accuracy in diagnosis of other musculoskeletal disorders using MSUS and to explore specific protocols for sonographers, to improve its reliability. In the hands of an experienced sonographer, MSUS is comparable in accuracy to MRI in assessment of rotator cuff pathology.
